1. APPLICATION METHOD

In accordance with the 1st paragraph of the 13th article of the KVK Law, you can send your request regarding the exercise of your rights specified in the 11th article of the same Law via a registered electronic mail (KEP) address or by using the electronic mail address you have previously notified to our Organization and which is registered in our system, to our address kvkk@dostcam.com.tr, or you can deliver it in writing in person, through a notary, or via registered mail with return receipt to our Organization's contact address. 2. INFORMATION REQUIRED IN THE APPLICATION;

a) Your name, surname, and if the application is in writing, your signature,

b) If you are a citizen of the Republic of Turkey, your T.R. identity number, if you are a foreigner, your nationality, passport number, or identity number, if any,

c) Your residential or business address for notification,

ç) Your e-mail address, telephone, and fax number for notification, if any,

d) Your request subject must be included. Information and documents related to the subject must be attached to the application.

3. THINGS TO KNOW BEFORE APPLICATION

Personal data owners defined as data subjects (Hereinafter referred to as "Applicant") in the Personal Data Protection Law No. 6698 ("KVK Law") are granted the right to make certain requests regarding the processing of their personal data in Article 11 of the KVK Law.

In accordance with Article 13/I of the KVK Law, applications to be made to our Organization, which is the data controller, regarding these rights must be transmitted to us in writing or by other methods determined by the Personal Data Protection Board ("Board").

It is important that the personal data we hold about you is accurate and up-to-date. Therefore, please notify us when there is a change in your personal data. In accordance with the KVK Law and relevant legislation, regarding yourself; 

  • You can learn whether personal data is processed or not,  
  • You can request details regarding the information we collect, 
  • You can learn the purpose for which this information was collected and how it is used, and the third parties to whom personal data is transferred domestically or abroad, or 
  • If it is incomplete or incorrectly processed, you can request their correction, updating, or deletion if the reasons requiring their processing have ceased to exist, including notifications to be made to third parties to whom the transfer was made.

In this context, applications to be made to our Organization "in writing" should be made by printing out this form and writing "Personal Data Protection Law Information Request" on the envelope or in the subject line;

  • With the applicant's personal application,
  • Through a notary or registered mail with return receipt, 
  • It can be transmitted to us by sending it to our Organization's registered electronic mail (KEP) address. 

4. DATA SUBJECT APPLICATION FORM

This Form has been prepared in accordance with the rights you have as a data subject under the Personal Data Protection Law No. 6698 ("KVK Law"), to fulfill your application request regarding your personal data at DOST CAM (Data Controller). After filling in the sections reserved for you to fill in the Form clearly and completely, you can send it with a wet signature to the contact person Dost Cam, at the correspondence address 10008 Sokak No:14 A.O.S.B Çiğli/ İzmir, or via electronic mail to kvkk@dostcam.com.tr.

Your application will be answered as soon as possible and within 30 days at the latest, in accordance with the KVK provision. We want you to know that as the Data Controller, we will contact you to conclude your application even if the information and documents you provide are incomplete or incomprehensible.
